The Evil Within, specifically the first game, is one of the best survival horror games in recent years. This game was brought to light by legendary Resident Evil creator Shinji Mikami. Pedigree aside, The Evil Within blends tense and challenging survival horror gameplay with a creepy, if sometimes bonkers narrative.
Much like the games that inspired it, namely, Resident Evil and Silent Hill, The Evil Within excels in world-building, character arcs, “jump off the screen” scary moments, and fantastic creature design. Yes, this game’s horror often comes down to the ghastly denizens you will encounter during its runtime. Just be sure to save up your ammo and health kits in order to survive the horror.
15 Zehn/Neun
Zehn and Neun are two hulking ogre-like creatures that act as guardians of the church in Chapter 6 of the game. These look monstrous creatures feature disgusting pustulous and growths all over their face. Zehn and Neun are actually twin brothers who seek to guard their domain as they see fit, wielding powerful clubs to deal massive damage.
14 Juli’s Doppelgangers
Juli’s doppelgangers actually don’t appear in the main game but are a part of the DLC, The Consequence. As their name implies, these doppelgangers are shadowy versions of Juli Kidman. Apart from looking quite spooky and taken right from a ghost story horror movie, these creatures can attack with bare hands or a large ax.
13 Sentinel
The Sentinel was once a traditional wolf but had the unfortunate pleasure of being experimented on by the horrific cult in the game. Like Zehn and Neun, the Sentinel was also a guardian of the church.
Disgustingly, this wolf creature feasted on the flesh of sacrificial victims which led to its unkempt and zombified appearance. It’s best to avoid this one if you don’t want to get chomped on.
12 Shade
It’s very hard to describe the Shade. The Shade is essentially a humanoid creature with a giant beacon light for a head. This monster also wears a bloody garment and clammers around with inhuman movements. The Shade is a relentless pursuer of Juli in both DLCs, the Assignment and the Consequence.
11 Trauma
Much like the Silent Hill series, The Evil Within is known for featuring creatures that are meant to be representations of one of the character’s psyche.
In the case of Trauma, a hulking brute of a character, it is supposed to represent the pain and turmoil Ruvik felt as his father forced his dangerous religion down his throat. One look at Trauma and you will want to run and hide somewhere safe.
10 The Keeper
The Keeper is probably the most recognizable creature from The Evil Within due to being featured heavily in the marketing lead-up to the original game’s release. The Keeper is first seen at the start of the game where it is butchering some bodies in its disgusting lair.
The Keeper is best known for its cage and barbed wire head. The Keeper is also very reminiscent of Pyramid Head from Silent Hill.
9 Doppelganger
The regular doppelganger characters are a similar version of the haunted creatures. These hooded and bloodied figures are essentially powerful clones of the main baddie Ruvik.
These semi-elite hunters feature psychokinetic powers due to their bond with Ruvik. The fact that they run around with no distinct face, only a dark socket where their face should be, makes them incredibly harrowing.
8 Cadaver
To explain the Cadavers is like trying to explain madness to someone. These humanoid creatures feature large boils all over their body. They also crawl around Reagan-style from the original Exorcist movie. What makes these creatures particularly scary is how they will transform to a more powerful stage if not dealt with right away.
7 Shigyo
The Shigyo monsters are found much later in the game and are essentially a combination of various bodies. These aquatic creatures are the unfortunate outcome of drowning during experimentation and testing.
Its most terrifying feature has to be its massive mount and rows of uneven teeth ready to gnaw into Sebastian’s flesh. Best to stay out of the water to avoid these aberrations.
6 Sadist
The Sadist is a massive brute that chases Sebastian down with a bloodied and rusted chainsaw. This description alone is enough to send a shiver down your spine.
The Sadist looks eerily similar to the chainsaw creature from the village in Resident Evil 4. The Sadist might look like the most “normal” enemy in the game but that doesn’t mean it isn’t downright terrifying.
5 Amalgam/Amalgam Alpha
Amalgam and its counterpart Amalgam Alpha is the final boss in The Evil Within and is the literal embodiment of hatred, anger, and pain that antagonist Ruvik holds deep within. This massive tentacled monstrosity is both otherworldly and familiar.
As you fight Amalgam you will feel like you have seen a boss like this in similar horror games. Both versions look similar but are equally disturbing to look at.
4 Quell
Quells are what would come about if an octopus and a human merged into one off-putting monster. Armed with a plethora of suction cups, the Quells can attach themselves to walls, floors, and ceilings in order to flank Sebastian. While the Quells aren’t pleasing on the eyes, they are most known for turning invisible.
3 Heresy
Heresy is essentially a giant spider that has gained power and form from absorbing and assimilating the minds of others from STEM. Heresy was meant to be just another minion-like fodder created by Ruvik but it ultimately took too much power and become sentient right away. Heresy is fast, powerful, and terrifying in equal measures.
2 The Haunted
The Haunted are your garden variety enemies in The Evil Within. You might think that garden variety equates to less scary, however, due to their human likeness, zombie-like movement, and downright disturbing masks, The Haunted are actually some of the most off-putting enemies in the game.

1 Laura
Hands down the scariest creature in all of The Evil Within is Laura. One look at Laura, specifically how she moves, and you will quickly be hiding under your bed in sheer terror. This perpetually stalking creature never gives Sebastian a break.
She is able to move with such frenetic energy and can even move through shadow portals to flank you during the many battles with her. However, fire might just do the trick to wipe her out for good…maybe.
