Its corpse was found by a powerful fey spirit, who carved its face from its body and turned it into a magical mask that allowed the wearer to transform into the Deku Scrub. The creation of this item might seem like a cruel choice on the part of its maker, but the spirit of the original creature lives on within the mask, and it can briefly glimpse the world through the eyes of the wearer whenever its powers are used.

Once a user has attuned to the Deku Mask, they can use an action to put it on, which will transform them into the Deku Scrub. The wearer remains in this form until they take the mask off or if they drop to zero hit points, which will cause the mask to fall off. The wearer’s original gear merges into their new form and they cannot access it without taking off the mask. Once they have taken on the Deku Scrub form, the wearer’s stats switch to the ones shown below. The transformation masks in The Legend of Zelda: Majora’s Mask were once living beings. Whoever wears the mask gains access to the abilities and appearance of the original being. The Dungeons & Dragons equivalent to the mask could also offer some interesting roleplaying opportunities by allowing the personality of the original creature to come through and merge with that of the existing character.

In the case of the Deku Scrub, the personality would be that of a child who spent its entire life living in the woods.
